Our Services

We provide treatment for chronic, short-term or non-emergency conditions.

Typical treatments include those for:

  • Acute conditions (flu, back pain, etc.)
  • Chronic conditions (hypertension, diabetes, etc.)
  • Work and sports physical exams
  • Mental Health
We are unable to offer these services:
  • On-site labs and x-rays
  • Gynecological exams
  • Immunizations
  • On-site prescription drugs
  • Narcotic pain medications
  • CDL physical exams

Our Volunteers

Medical services are provided by volunteer licensed physicians, nurse practitioners, physician assistants and nurses. Our volunteer spiritual care team and receptionists also provide care for our patients.

10:00am – 11:30am

Tuesday, Wednesday & Thursday
6:00pm – 7:30pm

Our Requirements

All care is provided free of charge (donations are gratefully accepted). There are no eligibility requirements. No income, insurance or geographic restrictions apply.

A donation of $20 is suggested for physical exams. Demographic statistical information is requested. Please be prepared to inform us of your medical history and current medications.

ENT Specialty Clinic

ENT Specialty Clinics are offered monthly as needed in conjunction with our Primary & Urgent Care Clinic. Preference will be given to patients referred through the CCHC Primary & Urgent Care Clinic. Patients are pre-scheduled for appointments in 15-minute increments. A limited number of walk-in patients may be seen as time allows. To schedule an appointment, leave a voicemail with our medical clinic manager at 260-250-4404.

Counseling Session Schedule

  • Counseling sessions are offered up to twice per month as needed in conjunction with our Primary & Urgent Care Clinics
  • You will be scheduled for a 30-minute counseling session

Patient Requirements

  • Counseling is available to adult patients throughout northeast Indiana and northwest Ohio
  • Services are offered free of charge
  • Donations are gratefully accepted
